اغلب افرادی که با این اختلال مواجه می‌شوند به‌نوعی در کودکی دچار آسیب‌هایی مانند بی‌توجهی و کمبود محبت یا مرگ یکی از والدین و یا طلاق والدین بوده‌اند. چنین آسیب هایی آنها را دچار استرس و اضطراب شدید از رها شدن کرده است. این افراد ترس های شدید مربوط به طرد شدن و رهاشدگی و خشم را با جدایی عاطفی تجربه می کنند.

روابطی ناپایدار با خانواده و دوستان خود دارند. ممکن است علاقه شدیدی به یک فرد پیدا کنند و ناگهان از آن شخص متنفر شوند. بسیاری از روابط عاشقانه آنها دائما با شکست روبرو است. چون هر گونه تغییر در رابطه را یک واکنش شدید احساسی تلقی می کنند. به این معنی که اگر به کسی علاقه داشته باشند و آن فرد از پیش آنها برود، احساس پوچی و انزوا می کنند.

روانشناسان علل اختلال شخصیت مرزی را به عوامل ژنتیک یا ارثی و تجارب سخت دوران کودکی ربط می دهند. خشم و ترس شدید در این افراد از تنها ماندن از دوران کودکی آنها سرچشمه می گیرد. این افراد حتی اگر تجسم کنند که تنها هستند، دچار ترس و عصبانیت می شوند.
